AI summary
Gives you a free age and appearance change, nigh-unbreakable willpower and enormous pain tolerance, strong resistance or outright immunity to madness and corruption such as the Great Ones' influence and to supernatural diseases like the Scarlet Rot, and a body at least at peak human capability able to wield huge weapons and fight enormous monsters. You are an exceptionally skilled warrior and monster slayer across swords, bows and firearms, have mastered the Art of Quickening for greatly increased speed and agility, have memorised every known Caryll Rune and its benefits, hold high skill and talent across FromSoftware's magic systems — pyromancies, miracles, sorceries and incantations — and pilot mecha well enough to take Armored Cores, Muscle Tracers and NEXTs into battle and beat stronger machines using museum pieces. Slain foes yield souls, runes and blood echoes to strengthen yourself either alone or more efficiently with a Maiden's help, powerful enemies may yield Remembrances granting special weapons, incantations and techniques, exceptionally powerful ones may yield Relics that enhance you once bound to a Chalice — you start with one — and you carry Gen 10 augmentations that further improve your piloting and can safely integrate Coral, which may leave Coral living in your head.

Effects
40You gain a free age and appearance change to whatever you want.
You have nigh-unbreakable willpower and are able to push through all manner of hardship and despair, and have also gained enormous pain tolerance.
You are incredibly resistant (if not outright immune) to madness and corruption, such as the Great Ones’ influence.
You are incredibly resistant (if not outright immune) to supernatural disease, such as the Scarlet Rot.
Your body has become peak human at the very minimum (before any magical or technological enhancements come into play), able to wield huge and heavy weapons and fight against enormous monsters.
You have become an incredibly skilled and talented warrior and monster slayer, with deep skill in many different types of weapons (swords, bows, firearms, etc.).
You have learned and mastered the Art of Quickening, greatly increasing your speed and agility.
You have memorized all known Caryll Runes, gaining their benefits such as improved stamina, fire resistance, poison resistance, etc.
You have access to the magic systems of FromSoftware settings (pyromancies, miracles, sorceries, incantations, etc.) and are highly skilled in such magics. You also gain a very high level of talent for magic in general.
You are an incredibly skilled and talented mecha pilot, able to pilot Armored Cores, Muscle Tracers, and NEXTs without problem and use outdated museum pieces to take on much stronger mechas.
Slaying foes will have you obtain souls, runes, and blood echoes that you can use to strengthen yourself (which you can either do on your own or more efficiently with the help of a Maiden or equivalent). The older and more powerful the slain foe is, the more you gain.
Slaying powerful foes may also reward you with Remembrances, echoes of their power that can be granted to you in the form of special weapons, incantations, techniques, and the like.
Defeating foes of particularly great power may also reward you with Relics, artifacts that will grant you some manner of enhancement once bound to a Chalice. You start with a single Chalice, but it’s possible to obtain/find/create more with luck or via defeating strong foes. Each Chalice can have up to 3 Relics bound to it, though you may swap them with only a little effort.
You have received Gen 10 Augmentations, increasing your piloting ability even more. Curiously, you may also safely integrate Coral elements into it, which may result in Coral being living in your head if you want.
Optionally, you have gained one or more of the following types of immortality of your choice. This also spreads bonfires, sites of grace, etc. as/where applicable.
The Darksign. You can never go Hollow.
The Grace of Gold.
The Dragon Heritage's Oath. This version doesn’t spread dragonrot.
Optionally, you have become a Shardbearer, gaining one or more Great Runes of your choice (Godrick’s, Radahn’s, Rune of Death, etc.) that grant you their powers.
Optionally, you gain one or more weapons of your choice from FromSoftware games (Moonlight Greatsword, etc.).
Optionally, you gain one or more armor sets of your choice from FromSoftware games.
Optionally, you gain one mecha (MT, AC, NEXT) of your choice. This may be customized as you wish.
Optionally, you may have an Erdtree appear in your world or a world you have self-inserted in. This also makes runes appear.
Optionally, you become one or more of these FromSoftware Player Avatars, gaining their memories, experiences, skills, abilities, relationships/bonds, and so on. How this works if you become multiple Player Avatars is up to you. If you do this and insert yourself into their works, you get self-inserted into them by default (because they are you). If you became the Chosen Undead, Bearer of the Curse, or Ashen One, you are fully alive and the Curse of Undeath is removed from you. This automatically includes some options:
The Slayer of Demons.
The Chosen Undead: Darksign.
The Bearer of the Curse: Darksign.
The Ashen One: Darksign.
The Hunter.
The Tarnished: Grace of Gold.
C4-621 Raven.
You may gain any FromSoftware characters of your choice as companions. By default you will quickly grow a bond with them. You may choose what relationships you will develop with each of these companions (being lovers, friends, etc.). If relevant (self-insert into their world, world merge, etc.), you are guaranteed to meet these companions in-universe, meeting them in positive circumstances (this resurrects companions dead by that point, which may draw attention). If you became a Player Avatar, you automatically gain these companions:
Slayer of Demons: Maiden in Black.
Chosen Undead: Anastacia of Astora.
Bearer of the Curse: Emerald Herald.
Ashen One: Fire Keeper.
Hunter: Plain Doll.
Tarnished: Melina, Ranni, Marika.
C4-621: Ayre.
Optionally, you may Self-Insert into any FromSoftware setting (Dark Souls, Armored Core, Elden Ring, etc.) or a variant (fanfics, etc.). You may choose when, where, and what world. The specifics are up you; maybe you get dropped in (default), maybe you insert yourself into or replace a certain character, maybe you get reincarnated, maybe you get merged with a local counterpart of yourself, etc.
